Our view of the service

Transforming Mind Solutions is a private independent clinic based in Northampton and run by Transforming Mind Solutions Limited. The service provides a range of treatment for mental health conditions, including depression, obsessive compulsive disorder and anxiety. Treatment options included Transcranial Magnetic Stimulation (TMS,) Transcranial Direct Current Stimulation (tDCS,) Nasal Esketamine, Oral Ketamine solution and Pharmacogenetic (PGx) Testing. The most utilised treatment was TMS. A typical treatment plan for TMS was 1 session a day, 5 days a week for 6 weeks, however this varied on an individual basis. Treatment was typically administered within the clinic, however tDCS could be self-administered at home. The provider worked collaboratively with another healthcare provider to offer patients treatment at satellite locations.

Care and treatment were provided to both adults and children (aged 14 and over). The service is for self-funding clients and those funded through private medical insurance. The service received self-referrals, and referrals from NHS staff or private doctors.

Transforming Mind Solutions registered with CQC on 23 August 2024 to provide the regulated activity of treatment of disease, disorder or injury. At the time of our assessment there was a registered manager in post. Transforming Mind Solutions also provides weight management treatment, however we were assessing the treatment of mental health conditions, so this did not fall within the scope of this assessment. The weight management element of this service falls within the scope of the Primary and Community Care inspection team.

This was our first comprehensive assessment of the service, across all key questions and quality statements. We rated the service as requires improvement.

We found the provider was in breach of 2 regulations in relation to Safe Care and Treatment (Regulation 12) and Good Governance (Regulation 17). We found the provider had not established effective systems and processes to assess, monitor and improve the quality and safety of the service. The provider had not ensured staff had the right skills, qualifications and experience to carry out their roles. Safeguarding training compliance was low, so we were not always assured people at risk of, or experiencing, abuse or neglect were protected from harm. We found the provider had not ensured staff were suitably trained and competent to manage and support with the self-administration of medicines. Medical records were not obtained from patients’ general practitioner (GP) prior to the prescribing of controlled drugs to make sure care and treatment remained safe. Policies and procedures lacked detail and did not provide staff with clear guidance in relation to staff training. The provider’s risk register did not reflect the concerns we identified during this assessment. This meant potential risks to people using the service were not identified and mitigated in a timely manner, which meant people were at an increased risk of harm.

We have asked the provider for an action plan in response to the concerns found at this assessment.

We found areas of good practice. Patients and carers were positive about their experiences with using the service. The facilities and equipment met the needs of people, were clean and well-maintained, and risks were managed through environmental risk assessments. Staff considered people’s communication and personal needs. Care and treatment were evidenced based and in line with best practice.
