This means we looked for evidence that the service met people’s needs.
At our last assessment we rated this key question good. At this assessment the rating has remained good. This meant people’s needs were met through good organisation and delivery.
Care and treatment were planned with people and adapted to their individual needs. Care plans were personalised and updated, and staff provided interpreting or other communication adjustments where required. People told us they were involved in their care planning and understood what care and treatment to expect, which supported choice and control. Information was clear, accessible and tailored to needs, and staff obtained consent for information sharing. Feedback was gathered through multiple routes, and actions were taken in response, such as reallocating staff and improving communication pathways.
The service understood local needs and worked to support continuity of care, including for prison leavers, and adapted approaches to improve engagement with specific community groups. Staff and leaders considered equity and inclusion in planning and delivery, and outreach supported people not accessing treatment.
However, records did not always confirm that people had received a copy of their recovery plan, reducing assurance that people could review their care independently. Planning for future care was consistent, and recruitment for a permanent aftercare practitioner was underway, with agency staff providing cover.
The service was looking at how it could improve continuity of treatment for prison leavers returning to its geography.
