During our inspection of this key question, we found that systems were in place, but were not operated effectively to assess, monitor and mitigate the risks relating to the health, safety and welfare of service users and others who may be at risk. This resulted in a breach of Regulation 17, Good governance. You can find more details of our concerns in the detailed findings below.

Governance, management and sustainability

Not all regulations met

The provider’s oversight of regulatory requirements required improvement.

Systems and processes were not embedded among staff which highlighted a number of issues which included, the control of substances hazardous to health, emergency medicines, radiography equipment maintenance, infection prevention and control and fire safety management.

There were effective processes for investigating incidents and accidents, and for receiving and acting on safety alerts. The storage of completed accident records did not comply with General Data Protection Regulation requirements.

The practice had systems and processes for learning, quality assurance, and continuous improvement. This included undertaking audits according to recognised guidance.

Concerns and complaints were responded to appropriately, and outcomes were discussed to share learning and for improvement.

Staff feedback was obtained through informal discussions. They were encouraged to offer suggestions for improvements to the service, and they said these were listened to and acted upon, where appropriate.

The practice gathered feedback from patients, the public and external partners, and responded accordingly.

The practice had taken steps to improve environmental sustainability. For example, staff ensured that lights were switched off when not in use. Clinical waste bins were used appropriately to avoid excess unnecessary incineration of recyclable material.

Staff went over and above to deliver good care and treatment. One example being was how staff supported older patients by walking them to their cars after treatment.
