Our view of the service

Assessment Date: From 26 August to 01 October 2026. This assessment was undertaken as a comprehensive assessment. The service was registered with CQC on 27 September 2024. This was the first assessment since the current provider, Filey Care and Support Ltd, was registered with CQC.

The service was rated inadequate (published 9 November 2023 and 25 July 2024) under the previous provider.

Two Rivers is a residential care home providing accommodation and personal care for people with a learning disability, autistic people, people with mental health needs and people with physical support requirements. They were supporting 8 people at the time of the assessment.

We assessed the service against ‘Right support, right care, right culture’ guidance to make judgements about whether the provider guaranteed autistic people and people with a learning disability respect, equality, dignity, choices, independence and good access to local communities that most people take for granted.

The new provider for the service had made improvements. The service was clean, well maintained and homely. People were comfortable, well-presented and were observed engaging in meaningful activities. Staff interacted positively with people, promoted choice and supported people to maintain relationships, practise their cultural and religious beliefs and access their local community.

Care records were comprehensive, person-centred and regularly reviewed. Risk assessments, healthcare and support plans reflected people's needs and were informed by relevant healthcare professionals. Staff had knowledge about safeguarding, individual risks and people's communication and support needs. Medicines systems were robust and the registered manager used audits and competency checks to maintain oversight.

There was evidence of strengthened leadership and governance since the current provider took over the service. Managers and staff consistently described improvements in communication, staff support, accountability, care planning and quality assurance. Governance systems included audits, supervision, training oversight and service improvement planning. Evidence showed that issues identified through incidents and audits were appropriately addressed and used to make improvements.

Our observations, discussions with staff and managers, record reviews and relative feedback demonstrated that improvements had been embedded into practice. The provider had addressed previous concerns relating to governance, communication, staff oversight, care planning and safeguarding through strengthened leadership, improved quality assurance systems and more effective oversight of care delivery.
