Assessing needs

Delivering evidence-based care and treatment
The provider had systems to ensure that people who used the service received person-centred care and treatment, that was appropriate, met their needs and was reflective of their personal preferences. Patients confirmed preventive oral health care was encouraged.
Patients confirmed their consent was obtained for treatment in line with legislation and guidance. Staff understood their responsibilities under the Mental Capacity Act 2005 and described how they involved patients’ relatives or carers when appropriate and made sure they had enough time to explain treatment options clearly.
Patients were referred to primary and secondary care specialists for treatment the practice did not provide. This included referring patients with suspected cancer under the national faster diagnosis pathways.
We looked at a selection of patient care records. The information recorded in patient care records was not in line with recognised guidance and were difficult for the provider to locate and interpret. They lacked detailed information about patients’ changing oral health and discussions to explain treatments and obtain valid consent.The records would not enable another clinician to easily understand a patient’s current state of health and the nature of any care that had been given. We discussed this with the provider and signposted them to audit tools and resources to support them to review and improve these.
The practice had systems for dispensing medicines in line with current guidance. We highlighted the records should also be improved to justify where antimicrobials were prescribed.
