Well-led
We found this practice was not providing well led care in accordance with the relevant regulations.
The provider had made insufficient improvements to put right the shortfalls and had not responded to the regulatory breach we found at our inspection on 26 November 2024. We have told the provider to take action. We will be following up on our concerns to ensure they have been put right by the provider.

Governance, management and sustainability
Staff and leaders told us of the systems in place to manage risks for patients, staff, equipment and the premises.
At the assessment on 29 April 2025 we found the practice had made the following improvements to comply with the regulation:
The team showed us that accidents and incidents were now recorded, reported and used for shared learning in accordance with practice protocols.
The system to ensure hazardous substances were appropriately managed had been improved.
The practice had reviewed their protocols and dental sharps were now managed safely.
The compressor was serviced and maintained according to manufacturer’s guidelines.
Decommissioned equipment was removed from circulation.
However, further improvements were still needed in the following areas:
We could not be assured the fire safety protocols were embedded. We noted the basement area around the electrical equipment had been cleared of potential fire risks and a domestic smoke detector had been installed. However, the fire risk assessment had not been updated to include the basement and identify and monitor the potential fire risks in this area. The practice emergency lighting had been serviced but further recommended action had not been taken.
There was a continuing high turnover of staff resulting in a lack of familiarity with the governance systems in place.
We found a lack of clear leadership and oversight across key aspects of practice management, including governance systems and safety processes. This compromised the provider’s ability to ensure well-led service delivery.
