Feedback from staff was obtained through meetings and informal discussions.

The majority of staff stated they felt respected, supported and valued. They were proud to work in the practice. Some staff did not always feel they were encouraged to offer suggestions for improvements to the service or that these were listened to or acted upon. We discussed this with the compliance manager who assured us that improvements would be made to the arrangements for staff involvement.

Staff told us how they collected and responded to feedback from patients, the public and external partners.

Systems and processes were generally embedded. However the assessment identified a number of areas where improvements could be made in relation to the overall management of the practice.

The information and evidence presented during the assessment was not consistently well documented.

The practice had a governance system which included policies, protocols and procedures that were accessible to all members of staff. Improvements were needed so that these were reviewed on a regular basis.

The practice had information governance arrangements and staff were aware of the importance of protecting patients’ personal information.

Staff password protected patients’ electronic care records, and paper records were stored securely and complied with General Data Protection Regulations (GDPR).

Some improvements were needed to the processes for identifying and managing risks, issues and performance.

The practice had systems to review and investigate incidents and accidents, and for receiving and acting on safety alerts.

The practice responded to concerns and complaints appropriately. Staff discussed outcomes to share learning and improve the service.

The practice had systems and processes for learning, quality assurance and continuous improvement. We found improvements were needed so that audits were carried out and used to make improvements in accordance with recognised guidance.
