We found this practice was not providing well-led care in accordance with the relevant regulations. We will be following up on our concerns to ensure the provider had made the required improvements.

During our inspection of this key question, we found a number of systems or processes were not operated effectively to assess, monitor and mitigate the risks relating to the health, safety and welfare of service users and others who may be at risk.

This resulted in a breach of Regulation 17, Good governance.

Governance, management and sustainability

The provider’s oversight of regulatory requirements required improvement.

Systems and processes were not entirely embedded which highlighted a number of issues that included the control of substances hazardous to health, data protection, disability equality, radiography equipment maintenance, and the management of fire safety.

Staff were aware of the importance of protecting patients’ personal information. Staff password protected patients’ electronic care records.

There were effective processes for investigating incidents and accidents, and for receiving and acting on safety alerts.

Concerns and complaints were responded to appropriately, and outcomes were discussed to share learning and for improvement.

Completed accident, incident and complaint records were not stored securely to protect personal data in line with General Data Protection Regulations.

The practice had systems and processes for learning, quality assurance and continuous improvement.

Staff had clear responsibilities, and systems of accountability to support good governance. Improvement should be made to ensure that policies and procedures reflect Market Place Dental Practice when templates are used.

Staff feedback was obtained through meetings and informal discussions. They were encouraged to offer suggestions for improvements to the service, and they said these were listened to and acted upon, where appropriate. For example, music was installed in the decontamination room, and new uniforms were supplied for clinical and reception staff.

The practice gathered feedback from patients and responded accordingly.
For example, filtered drinking water was provided.

The practice had taken steps to improve environmental sustainability. For example, staff recycle where possible, separate the plastic from any gypsum models for disposal, and turn off the lights and electrical equipment when not in use.

Staff made courtesy calls to patients following invasive treatment. They send sympathy cards when a patient passes away and assist patients up and down the stairs when required.
