Our view of the service

Lyme Regis Care Home with Nursing is a residential care home providing accommodation for up to 27 people who require nursing or personal care. The service provides support to older people, some of whom live with dementia, and people who have a physical disability. People in care homes receive accommodation and nursing or personal care as a single package under one contractual agreement. At the time of our inspection there were 26 people using the service.

Accommodation is provided over 2 floors. There are 2 lounges located on the upper floor, main lounge including dining area in the centre and a smaller quiet and visitors lounge, accessible to all people. Both floors are accessed by a lift and stairs. There are outside spaces for people to enjoy, and all have level access.

The assessment took place between 22 July and 13 August 2025. We visited the service on 22 and 23 July 2025. During the inspection we assessed the quality statements under all key questions of safe, effective, caring, responsive and well led. We inspected this service partially because of concerns received about the quality and safety of the care provided in the home and as a newly registered service under a new registration on 2 April 2024. New services are assessed to check they are likely to be safe, effective, caring, responsive and well-led.

Potential risks in the care environment were not always detected and effectively mitigated. The safety of the premises, communal and personal spaces (such as bedrooms), and the living environment were not always checked and managed to support people to stay safe.

The provider did not follow national guidance in relation to infection control. The premises the provider was responsible for were not clean and hygienic.

People did not always receive their medicines in a safe way, and as prescribed for them.

Assessments of people’s needs were not always comprehensive, and people’s care plans had not always considered the full range of their diverse needs, including psychological and emotional well-being. Different parts of people’s care and treatment were not efficiently coordinated to ensure consistently positive outcomes for them.

Processes for monitoring the effectiveness of people’s care, treatment and support were not always effective in ensuring actions were taken to continuously improve the service.

Consent to care and treatment and subsequent best interest’s decisions had not always been obtained in line with legislation and guidance. People were not always involved in consent decisions as much as possible, nor given the appropriate information, support and time needed to make an informed decision.

People’s care plans were focused on people’s medical needs and lacked person centred details. Care plans lacked information about individual’s personal life history, wishes and preferences, passions and hobbies, cultural background and spiritual beliefs.

At this inspection we found the evidence to support the rating of requires improvement. We found 2 breaches of the legal regulations in relation to safe care and treatment and governance.

People's experience of this service

People and relatives expressed mixed views about the service they received at Lyme Regis Care Home with Nursing and felt the service provided was not consistently safe. Comments from relatives included, “I think [my loved one] feels safe most of the time. [My loved one] is bedbound and [they] don't ring the call bell because staff don't come quickly. [My loved one] says they take ages to come. Sometimes it's 3 or 5 minutes, but other times it's closer to 20 or 30 minutes”, “There's nothing to stimulate [my loved one] at all there. [They] just stare at the walls all day. There's no newspaper. I bring the crosswords or the book for [them]. There's no religious input at all. [My loved one] has a very dull existence” and “I'm very happy with the way my [loved one] is being looked after, extremely pleased with it. The food is beautiful, everything about the place is excellent.
