We assessed 2 quality statements for this key question. We found the service had made sufficient progress to address the regulatory breaches found at our previous assessment. Qualified practitioners were now carrying out the secondary reception screen in line with national guidance and care planning had improved.

At our previous assessment we found that care plans varied in quality for patients with a long-term condition and for patients with addictions and those who misused substances. They were not always completed promptly, and they were not always personalised or clearly discussed with the patient.

​During this inspection, we found that staff had completed care planning training, with some staff also undertaking additional training on the quality assurance of care plans. This had supported the provider in strengthening the quality and consistency of care planning practices. Quality assurance processes were in place and were supported by regional leads, providing additional review and governance arrangements. The provider continued to develop and embed its approach in this area.

The care plans we reviewed demonstrated patient involvement and contained relevant and personalised information. However, not all care plans included documented review dates, which limited assurance that they would be evaluated and updated consistently.

Of the 37 care plans required for patients with long-term conditions, 27 had been completed at the time of our inspection. The remaining patients had planned appointments in place to support the completion of their care plans.
