Learning culture

Score: 3

The provider had a proactive and positive culture of safety, based on openness and honesty. Staff listened to concerns about safety and investigated and reported safety events. Lessons were learnt to continually identify and embed good practice.

Staff were encouraged to reflect on their practice during team meetings and spot checks, and they shared examples of adapting care when approaches did not work. Records showed audits were completed, and although some could be more robust, actions were taken to address issues such as falls and medication errors. Staff said they felt comfortable raising concerns and were confident these would be acted upon.

Safe systems, pathways and transitions

The provider worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services.

One professional told us, “Communication is really good, frequent and timely."

Safeguarding

Score: 3

The provider worked with people and professional partners to understand what being safe meant to them and the best way to achieve that. Staff concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect. The provider shared concerns quickly and appropriately.

People told us they felt safe with staff, and family members confirmed they had no safeguarding concerns. One person said, “They make me feel safe.” A relative said, “I think they are safe with the carers.”

Staff understood safeguarding procedures and were confident concerns would be taken seriously. They described clear reporting routes and escalation processes, including contacting the local authority or CQC if needed.

Involving people to manage risks

The provider worked with people to understand and manage risks by thinking holistically. Staff provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them.

Care plans showed people were involved in decisions about their care, and staff respected choices such as routines and preferences. Risk assessments had been carried out to make sure people could receive care and support with minimal risk to themselves and others.

We raised one issue about a person’s personal emergency evacuation plan with the registered manager, and they took immediate action to address this.

Safe environments

The provider detected and controlled potential risks in the person’s home environment. They made sure equipment, facilities and technology supported the delivery of safe care.

Environmental safety checks were completed before care packages started. Staff demonstrated awareness of fire safety and other environmental risks.

Safe and effective staffing

Score: 3

The provider made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care that met people’s individual needs.

Staffing levels were sufficient, and rotas allowed time for rest between shifts. Recruitment checks were mostly robust, although one new employee’s employment dates and references required further verification. Following our site visit the registered manager carried out further checks and provided evidence of a safe recruitment.

Staff received thorough inductions and regular supervision. Training compliance was high, and staff felt supported to develop skills, including opportunities for vocational qualifications. Staff told us the training they received had been good quality and given them the confidence to carry out their role.

Infection prevention and control

The provider assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly.

Staff had access to personal protective equipment (PPE) and confirmed they followed infection control procedures.

Medicines optimisation

Score: 3

The provider made sure that medicines and treatments were safe and met people’s needs, capacities and preferences. Staff involved people in planning, including when changes happened.

Medicines were administered safely, and medication administration records were completed accurately. Staff were trained and assessed as competent in administering medicines. One person told us, “They [staff] are very good with tablets.”

Medicines audits were completed, although improvements were identified to make them more detailed. Before the end of the assessment the registered manager sent evidence of improved audits.
