Well-led
Well-led – this means we looked for evidence that service leadership, management and governance assured high-quality, person-centred care; supported learning and innovation; and promoted an open, fair culture. At our last assessment we rated this key question as good. At this assessment the rating has remained as good. This meant the service was consistently managed and well-led. Leaders and the culture they created promoted high-quality, person-centred care.

The provider had a shared vision, strategy and culture. This was based on transparency, equity, equality and human rights, diversity and inclusion, engagement, and understanding challenges and the needs of people and their communities.

Governance, management and sustainability
The provider did not always have clear systems of accountability and good governance. Systems and processes were in place to monitor the quality and safety of the service and managers completed a range of audits and checks. However, these processes were not always effective at ensuring improvements were fully embedded.
For example, the concerns we identified with medicines management had previously been identified in an external quality audit. Whilst action had been taken, we found improvements were not embedded. Management audits had not always identified gaps or inconsistencies in care plans, and management oversight of environmental and fire safety checks required strengthening.

Learning, improvement and innovation
The provider focused on continuous learning, innovation and improvement across the organisation and local system. They encouraged creative ways of delivering equality of experience, outcome and quality of life for people.
