Our view of the service
Date of assessment was 03 September 2026 to 16 September 2026. Unique Community Services - Leeds is a supported living service that provides care within people’s own homes. It supports older adults, younger adults, people with learning disabilities and autistic people. At the time of this assessment, the service was supporting 3 people. We carried out this assessment as the service had not yet been rated and we had received information of a recent incident of concern from a member of the public.
We assessed the service against the principles of right Support, right Care, right Culture to determine whether people with a learning disability and autistic people were treated with dignity, respect and equality, and were supported to make choices, maintain independence and access their local communities. We found these principles were being promoted and applied consistently across the service.
Both the management team and staff demonstrated a clear understanding of people’s individual needs and ensured that care and support was delivered in a person-centred manner.
Care plans were reviewed and updated regularly and contained sufficient information to guide staff in delivering support in line with current best practice. Staff worked collaboratively with people to ensure activities were undertaken safely and in accordance with their individual needs, preferences, and risk assessments. People and their relatives were actively involved in the development and review of care plans and were supported to understand these in ways that met their communication needs.
Feedback from healthcare professionals was positive and reflected effective partnership working. Both the management team and staff were kind, caring, and spoke positively about people and recognised them as individuals.
Staff had received appropriate training to ensure they were competent and confident in carrying out their responsibilities.
The management team and staff demonstrated a strong commitment to maintaining a high standard of care and to continually improve outcomes for people, with a clear focus on enhancing their quality of life.
