Safe – this means we looked for evidence that people were protected from abuse and avoidable harm. At our last assessment we rated this key question requires improvement. At this assessment the rating has changed to good. This meant people were safe and protected from avoidable harm.

Learning culture

Score: 3

The provider had a proactive and positive culture of safety, based on openness and honesty. Staff listened to concerns about safety and investigated and reported safety events. Lessons were learnt to continually identify and embed good practice. Staff told us leaders encouraged staff to attend training to improve and enhance practice.

We saw leaders had reviewed systems and processes since the last inspection to ensure they were effectively learning from safety events. People and their relatives said they were confident leaders would address and learn from issues they identified.

Staff shared they would pass any concerns back to the registered manager for them to address and were confident concerns would be acted upon appropriately.

 

 

Safe systems, pathways and transitions

The provider worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services.

During the last inspection we found there were areas where partnership working with other professionals could be strengthened. At this inspection leaders explained the steps they had taken to strengthen these relationships and improve transitions for people between services, including supporting people in hospital settings. People told us they felt they were supported well by staff. Partners also confirmed that these improvements now ensured smooth transitions for people.

Safeguarding

The provider worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. Staff concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect. The provider shared concerns quickly and appropriately.

Leaders had a clear understanding of safeguarding issues and were confident to address any issues appropriately. Staff we spoke to could identify what may be a safeguarding issue and could describe what steps they would take to ensure they kept people safe.

People and their families told us they felt care was safe and that they were confident they raised concerns they would be appropriately addressed by leaders.

Involving people to manage risks

The provider worked with people to understand and manage risks by thinking holistically. Staff provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them.

Peoples’ risk including those relating to medicine management and environmental risks had been assessed by the provider. Records demonstrated the provider liaised with professionals where they identified risks. People and their relatives felt they were supported safely. One relative told us, “They have worked out what to do to keep them safe”.

 

Safe environments

Score: 3

The provider detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care. The provider had ensured staff had received appropriate training. They also assessed risks to staff relating to supporting people within their own home and had also considered risks to people. We saw risk assessments relating to fire and moving and handling were being appropriately recorded.

Safe and effective staffing

The provider made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care that met people’s individual needs.

During the last inspection we found recruitment checks did not always obtain sufficient and appropriate references for new staff. At this inspection the provider had addressed this and records demonstrated staff were being recruited safely.

There were enough staff to support people. Staff told us the received good support from leaders and had regular supervisions. They also had yearly appraisals.

Staff shared that they felt the manager was approachable and supportive. People told us staff were “very good” and “knowledgeable”.

Infection prevention and control

Score: 3

The provider assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly. The provider had good oversight and management of personal protective equipment and ensured staff could access this as required. Staff had been trained in using and managing personal protective equipment. People told us they saw staff appropriately washing their hands and wearing gloves and aprons as needed.

Medicines optimisation

Score: 3

The provider made sure that medicines and treatments were safe and met people’s needs, capacities and preferences. Staff involved people in planning, including when changes happened.

During the last inspection the provider had introduced a new system for administering medicines, which had not had time to demonstrate effectiveness. At this inspection, we saw this system had been embedded and was effective.

People told us staff supported them with medicines in the way that suited them and that they felt staff were competent when helping them. Staff could describe how they administered medicine for different people following the appropriate guidelines.
