Learning culture

Score: 3

The service had a proactive and positive culture of safety, based on openness and honesty. Actions had been taken to investigate all incidents and accidents. Although there was no formal analysis to assess patterns and trends and ensure future risks were mitigated, this had little impact for people. Staff told us that when incidents and accidents occurred, any learning from these was shared with them through staff meetings and individual supervision meetings. People and relatives raised concerns, and the provider apologised when things went wrong.

Safe systems, pathways and transitions

The service worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services. The relative of a person who had a live-in carer told us, “I was worried the first time [carer] went on a break, but it was seamless.” Another relative told us their parent required complex care and felt that carers recognised when urgent support was needed or when they needed to be called to give medicines.

Safe environments

Score: 3

The service detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care. People told us their carers tidied up after care was provided to make sure their home was safe and there were no trip hazards. A person told us, “I do feel safe.” Before starting a new package of care an environmental risk assessment was carried out to identify potential hazards in people’s homes. Where appropriate, steps were taken to minimise any risks. Support plans clearly stated how care staff should enter each premises and, where key safes were used, details were only available to those who needed this information.

Infection prevention and control

Score: 3

The service assessed and managed the risk of infection. A relative told us, “They have to wear gloves to give one of my medicines and they always do.” Another relative told us, they go out of their way to help, they do the wash up and put clothes in the machine, yes, they go above and beyond.” Another said, “They wear masks if they have so much as a sniffle, otherwise they don’t need to. They wear gloves, and they are very particular about changing them between the personal care and making my meals.” Care staff had been trained in infection prevention and control and had access to personal protective equipment (PPE). Staff knew when and how to use PPE correctly and how to prevent the risk of infection. Spot checks included observational checks of the correct use of PPE.
