At our last assessment we rated this key question Good. At this assessment the rating has remained Good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

How staff, teams and services work together

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services.

Since the last inspection improvements had been made regarding feedback from staff in relation to communication with leaders.

 

There was good communication and joint working between the service and professionals. We received positive feedback from people and professionals. One professional told us, “[Manager] has been proactive in sending requested documents through in a timely manner and has demonstrated that continued improvements are being made and reviews and audits are being carried out.” Staff told us they communicated well with each other and leaders. A member of staff said, “You get sickness sometimes and it can be stressful. No day is ever perfect, but we work well together to get things done. As long as the residents are safe that’s what matters.” Paperwork we saw demonstrated collaborative working, with staff and professionals sharing information about people and their needs.

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves.

Since the last inspection improvements had been made in respect to staff routinely recording care practices, such as repositioning charts.

People experienced positive outcomes and had planned goals for the future. Feedback from staff confirmed people’s care and progress were monitored. Staff were knowledgeable and supportive of people's goals and wishes. A relative told us, “I really can’t fault them, they got my [relative] walking again, they’ve really helped her independence.”

Processes were in place to ensure people’s outcomes were monitored, and any changes were implemented. This was evidenced in people’s care plan reviews. Care plans were audited to ensure regular reviews took place and any actions were followed up. Records showed that staff recorded relevant care interactions accurately, such as when people were repositioned to reduce their risk of pressure damage.
