Our view of the service
Date of Assessment: 10 and 12 November 2025. Lindley Group Practice is a GP practice, located in Huddersfield, West Yorkshire, which delivers services to approximately 10,400 patients under a contract held with NHS England. The National General Practice Profile states that the practice demographic is 75% White, 16% Asian, 4% Mixed, 3% Black and 2% Other. Information published by the Office for Health Improvement and Disparities shows that deprivation within the practice population group is in the seventh decile (7 of 10). The lower the decile, the more deprived the practice population is relative to others. This assessment considered the demographics of the people using the service, the context the service was working within and how this impacted service delivery. Where relevant, further commentary is provided in the quality statements section of this report.
We carried out this assessment due to the practice changing their registration status with the Care Quality Commission (CQC) and currently not having a published rating. This was an announced assessment looking at all the quality statements and all key questions. At this assessment we found:
Safe: The practice had a good learning culture, staff could raise concerns, and managers investigated incidents thoroughly. There were systems and processes in place to support medicines management. A review of patient clinical records found that overall patients’ medicines and treatment were safely managed by the practice. However, we found some gaps in systems and processes around safe and effective staffing and infection prevention and control (IPC) which could potentially impact on patient safety.
Effective: We looked for evidence that staff involved people in decisions about their care and treatment and provided them with advice and support. The practice ensured that patients were involved in assessments of their needs and staff made sure they understood their care and treatment, to enable them to give informed consent. Staff reviewed assessments taking account of their communication, personal and health needs and involved those important to patients, supporting their best interests where they did not have capacity. A review of patient clinical records found that overall patients with long-term conditions were safely managed by the practice.
Caring: Patients were treated with kindness and compassion. Staff demonstrated how they protected patient privacy and dignity, treated patients as individuals and supported their preferences. This included cultural, communication and accessibility requirements. Outcomes from the most recent National GP Patient Survey were positive and demonstrated patients felt listened to, supported, and involved in decisions about their care. The practice supported staff wellbeing and feedback from staff about working at the practice was positive.
Responsive: We looked for evidence that the service met patient’s needs, and that staff treated people equally and without discrimination. Patients were involved in decisions about their care. The practice provided information in a manner people could easily understand. Patients had access to appointments, based on clinical need, and vulnerable people or those with protected characteristics were able to access care and treatment in ways that met their personal circumstances. The National GP Patient Survey outcomes were below average for some outcomes in relation to access. The practice took complaints and other patient feedback seriously and learnt from them to improve the quality of care.
Well-Led: We looked for evidence that the practice’s leadership, management and governance assured high-quality, person-centred care; supported learning and innovation; and promoted an open, fair culture. Leaders and staff had a shared vision and culture based on listening, learning and trust. Leaders were visible, knowledgeable and supportive, helping staff develop in their roles. Leaders demonstrated a commitment to continuous improvement, using feedback from people using the service, families, and staff to inform service development. The practice was a training practice and demonstrated a strong education and learning ethos. The practice engaged positively in the assessment process and was responsive to feedback.
We have rated this service good overall and good for all the key questions except the key question safe, which has been rated as requires improvement, due to a breach of regulation in relation to Regulation 12 – Safe care and treatment. We have asked the provider for an action plan in response to the concerns found at this assessment.
People's experience of this service
The practice encouraged and used feedback from patients who used the service to make improvements. As part of this assessment, we reviewed patient feedback data available from the National GP Patient Survey (2025) and the NHS Friends and Family Test (FFT).
Feedback from the National GP Patient Survey (2025) in relation to kindness, compassion and dignity, were broadly in line with national averages. We saw that 86% of patients felt the healthcare professional was very good or fairly good at treating them with care and concern (national average 86%), 87% felt the healthcare professional was very good or fairly good at listening to them (national average 87%), 94% had confidence and trust in the healthcare professional they saw or spoke to (national average 93%) and 96% felt they were involved as much as they wanted to be in decisions about their care and treatment (national average 91%).
Feedback regarding access was below the national average. We found 38% responded positively to how easy it was to contact the practice by phone (national average 53%) and 65% responding positively to the overall experience of contacting the practice (national average 71%).
The FFT data collected by the practice for the period November 2024 to November 2025, of which there were 1,395 responses, showed 77% of patients found the practice to be very good or good.
