The provider had a proactive and positive culture of safety based on openness and honesty. Staff reported incidents but were unaware of the outcome from the investigation that took place. Despite information being made available to staff they reported being unaware of learning from incidents inside the unit and in the wider company. However, other systems and processes were in place to ensure the safety of patients and protect them from avoidable harm.
