Our view of the service

We inspected Weaver Lodge from 30 June to 1 July 2026.

Weaver Lodge has been registered with CQC since December 2010 to deliver the regulated activities: Assessment or medical treatment for persons detained under the Mental Health Act 1983 and Treatment of disease, disorder or injury. The service had a registered manager.

The service is a 20-bed ward, which includes 4 self-contained bedsits.

At this inspection we assessed 1 assessment service group; long stay or rehabilitation mental health wards for working age adults where we inspected 33 quality statements.

We rated the long stay rehabilitation mental health wards for working age adults as requires improvement.

The electronic care record system lacked consistency and was difficult to navigate. There was a lack of crisis planning in place, in one care record a risk assessment had not been completed, and patient voice was not consistently present in care records. This meant that staff could not be quickly assured of patient risk or whether patients were involved in developing care and treatment. This was a breach of Regulation 12: Safe Care and Treatment.

The Oliver McGowan tier 2 training (nationally recommended training on meeting the needs of people with a learning disability and autistic people) was at 54%. This training was introduced in 2022 to improve the knowledge, skills, and culture of staff when providing care for patient with learning disabilities and autism. As the service supports people with learning disabilities and autism, there was limited assurance of how the needs of patients with a learning disability and autism were being met. This was a breach of Regulation 12: Safe Care and Treatment. We have asked the provider for an action plan in response to the concerns found at this assessment.

The service did not have a psychologist as part of the MDT, which is against national standards and NICE guidance for a rehabilitation service. Although the service told us they did not admit people who needed increased psychology input, however this was not clear in their admission criteria. This is a breach of Regulation 18: Staffing.  We have asked the provider for an action plan in response to the concerns found at this assessment.
