There was a clear framework of what must be discussed at team or directorate level in team meetings to ensure that essential information, such as learning from incidents and complaints, was shared and discussed.
Staff had implemented recommendations from reviews of deaths, incidents, complaints and safeguarding alerts at the service level.
There were insufficient systems in place to ensure that medicines audits and supervision took place. However, there were sufficient systems in place to ensure that staff had the required competencies to work on a care package.
