The service worked with people to understand and manage risks by thinking holistically. They provided care to meet people’s needs that was safe and supportive

At the last inspection, we found that there was a significant backlog of patients’ clinical notes from their previous GP practices’ which required summarising. This amounted to approximately 2,500 sets of notes which required summarising dating back to 2022, creating potential risk to safe prescribing and continuity if care. Since the last inspection the practice has taken a number of steps to address the issue and reduce the risks identified.

We found that the practice now had an increased capacity and resilience as there were additional staff allocated to support summarising and workflow. There were six monthly audits introduced to identify and address any unsummarised patient records.

There was now a new patient registration process where patients were offered a health check on registration, ensuring their medical history was collected promptly where there were gaps. Finally, the practice has ensured there was clinical oversight through a multi-GP review process where any unsummarised records were reviewed by more than one GP to identify actions and next steps.
