Our view of the service

The inspection of Marham House Care Home took place between 30 January and 5 March 2025. We visited the service on 30 January 2025. This is the first assessment for this newly registered service following a change of provider and CQC registration. Therefore, we inspected all quality statements under the key questions of safe, effective, caring, responsive and well-led. Marham House Care Home is registered to provide personal care and support for up to 66 people. At the time of the inspection, the service was supporting 53 people, including people living with dementia. This inspection was prompted in part by concerns received about a lack of consistent leadership, people’s safe care and whether staffing levels and staff skill met the needs of people using the service safely. There had been a number of changes of manager at the service and at the time of our inspection there was no registered manager and the post was vacant. As a result, the regional manager based themselves from Marham House Care Home full time and the district manager visited frequently. They told us they had taken this action, whilst recruiting for a full-time manager, to ensure continuity of management oversight for the service.

During this inspection we found improvements were needed to the leadership and governance and a breach of the regulations relating to good governance. We also found shortfalls around the management of medicines, improvements needed to the management of risks associated with people’s care and well-being and the safe management of the environment. This was a breach of the regulations relating to safe treatment. We did not find any shortfalls relating to the level of staff, but the leadership and deployment of staff required improvement. The regional manager and district manager for the provider were transparent and responsive in response to any concerns and shortfalls we identified and were committed to making any improvements necessary.

People's experience of this service

Overall, the majority of people and relatives were happy with their care, including the care and supported they received and how it was managed. However, some people and many relatives raised concern about poor communication from the management and the lack of information shared with them about the management changes. The provider said they would take immediate action to address this. Some people raised concerns there was not enough staff to support them and the length of time they had to wait for their care needs to be met.
