We looked for evidence that service leadership, management and governance assured high-quality, person-centred care; supported learning and innovation; and promoted an open, fair culture. At our last assessment, we rated this key question as requires improvement. At this assessment, the rating remains the same.

Shared direction and culture

Capable, compassionate and inclusive leaders

Score: 1

Leaders did not have capacity, capability or were knowledgeable enough to ensure risks at the service were well managed.

Policies were not developed or followed. There was a lack of understanding of regulations or best practice guidance.

However, staff told us via questionnaires that they felt supported by the managers at the service. They said there was a good positive atmosphere. Their feedback was sought, there was an open-door policy, and suggestions had been taken on to improve the service. The service had social events for staff wellbeing.

Freedom to speak up

The staff at the service and leaders told us they had a positive culture where people felt they could speak up and their voice would be heard.

The service had a whistleblowing policy.

Staff were aware of how to raise concerns. Overall staff felt their views and suggestions were actively taken on board and gave us examples of this.

Workforce equality, diversity and inclusion

Policies and procedures to promote diversity and equality were in place. The policy stated that all staff would receive equality and diversity training, however only the clinical staff had received this.

Governance, management and sustainability

There was no clear oversight of governance arrangements to ensure risks to service users, staff and visitors to the service were considered, managed and mitigated appropriately.

Infection control arrangements were inadequate. We advised the service of our urgent concerns following our site visit in March 2025. When we re-visited in June 2025, we saw that arrangements had been improved, however governance of this was still poor.

Service polices did not always contain best practice guidelines and sometimes they were not followed.

There was no significant event policy.

Clinical record keeping was poor or incomplete. Facilities, equipment and technology were not always well-maintained

The service was not displaying their current CQC rating within the premises or on their website. We informed them this was a legal requirement at our first site visit in March 2025. The situation had not changed when we visited the service again in June 2025.

Learning, improvement and innovation

Score: 2

The service had some focus on learning and innovation. However, client feedback on surgical procedures carried out was not evaluated. There were either no audits of the service or the ones we saw were limited and did not demonstrate they had led to improvements in patient care or outcomes.

The provider told us that the service had a green policy, it was part of their quality improvement plan. They wanted to make the service more environment friendly and were using paperless notes, reducing printing and recycling. However, the paperless notes we viewed were of poor quality and incomplete.

Staff feedback told us that staff felt they were supported to continuously learn and develop.
