We assessed 1 quality statement for this key question.The provider had not made sufficient progress to address the regulatory breaches found at our previous assessment.
We found that triage arrangements had improved and worked well, there was also improved structure to ensure Care Programme Approach reviews took place at regular intervals. However, careplans and risk assessments lacked clear goals linked to risks to the individual patient.

Patients’ needs were triaged well. At the last inspection staff had not consistently completed triage records for individual patient mental health needs and there was no multi-disciplinary review. During this inspection we found triage records were completed thoroughly by the mental health team. A daily meeting also took place each weekday morning where staff disccused patients who had been triaged as well as new referrals into the service.

Staff had not consistently developed risk assessments or care plans that met patients’ individual mental health needs. At the last inspection we found that staff had not completed risk assessments or care plans for patients. During this inspection we found that whilst risk assessments and care plans were completed, they lacked sufficient detail. It was not always clear what the risks were in relation to the patients' mental health needs. Outcomes, goals and interventions were not clearly defined in care plans. This meant that there were no meaningful interventions provided to patients who had been assessed as requiring support from the mental health team.

Staff saw patients for their care programme approach (A CPA is a package of care for patients with severe mental health problems) at agreed intervals. At the last inspection the provider had a system in place for oversight of CPAs and when patients had been invited, but this had not been implemented. During this inspection we found there was a system in place to ensure oversight of CPA attendance.
