Our judgement

People had been put at risk of not always having enough staff available to meet their complex needs.

User experience

Staff told us that the lower than needed staffing levels had been a common feature over recent months. They said they had needed to keep most of the staff in the home in case they were needed to respond to people becoming anxious or aggressive. This had limited the opportunities some people living in the home had to take part in activities in the home and community.

Other evidence

The manager told us that the usual staffing level during the day was six but since a new flat was built and a seventh person moved in he had aimed to have seven staff on duty. He had assessed that the previous night time levels, of two waking night staff and one senior sleeping-in, were still adequate.

The rotas we saw from February 2011 showed that on most day shifts there were six staff on duty. We sampled the rota between 18 June and 1 July 2011. During these two weeks there had only been two days when seven staff had been on duty. On other days there were six staff but on four days there had only been five staff on after 6pm. Several serious incidents had occurred at meal times and staff told us that the evening meal was not always finished by 6pm.

One person in the home was assessed as needing support by two male workers when upset and when out in the community. The manager told us he had appointed a new female permanent worker and a female relief worker recently. A permanent male worker had left in February but two men appointed to take his place had both withdrawn before starting. This had meant that one team had only one male worker. The other team had two males but across the service there was a shortage of male workers to cover shifts and rota gaps due to sickness or annual leave.
