Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them.

The service had effective systems to assess and identify people’s needs before they offered support. The registered manager met with people and their relatives as part of the assessment process and transition plans had been collaboratively developed. Information gathered during the assessment process formed the basis from which people’s care plans were developed.

Delivering evidence-based care and treatment

The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards.

Staff provided person centred care and respected people’s skills and knowledge. Care plans reflected people’s wishes and interests and staff told us, “We do tailored support around people’s needs and we design the care plans around the person”.

How staff, teams and services work together

The provider always worked well across teams and services to support people. They shared thorough assessments of people’s needs when they moved between different services, so people only needed to tell their story once.

The service worked effectively with health professionals and shared information accurately and promptly when requested. The provider’s director told us, “We are able to have open and honest relationships with partners. We have always promoted partnership working. Those open relationships benefit us, we’ve had some really big success stories”. Professionals confirmed the service worked effectively in partnership with them saying, “We have found them to be open to suggestions and recommendations”.

Supporting people to live healthier lives

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support.

Staff encouraged and supported people to exercise regularly and access healthy dietary options. People enjoyed going for walks with their support staff and records showed people had been regularly supported and encouraged to access the local community.

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves.

The registered manager visited people in their own homes regularly to monitor how staff provided support and enabled people to raise any issues with them directly. Care records were audited and checked by managers to ensure care had been provided as planned. Advice and guidance provided by involved professionals had been adopted and acted upon.

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment.

Staff sought consent before providing assistance and respected people’s decisions and choices. We observed people were able to change their minds in relation to planned activities and that staff respected and acted upon people’s decisions. Professionals were complimentary of the service’s approach to support meaningful decision making by people and told us, “The service are very inclusive in allowing [Person’s name] to make choices and decisions which has seen a marked improvement in their wellbeing and quality of life”.
