Cygnet Hospital Woking

Our overall rating of safe at Cygnet Woking has changed to requires improvement. Acute wards for adults of working age were rated good. Forensic wards were rated as required improvement. The service was in breach of legal regulation as staff did not always ensure that the ward environments were clean and well- maintained. We observed visibly unclean areas in ensuites and bedroom spaces which were either in need of repair and redecoration due to chipped and flaking paint or stained walls. Medicines were not always managed in line with national guidance or legislation. Staff did not always complete thorough risk assessments of all the wards and communal areas and therefore did not always remove or reduce any risks that should have been identified. There was an increased risk that patients could be harmed. We found staff were not fulfilling their duties in terms of enhanced observations due to sleeping while on observation. We found blind spots in patients’ bedrooms which were not sufficiently mitigated.

Requires Improvement: This meant that some aspects of the service were not always safe and there was limited assurance about safety. There was an increased risk that people could be harmed.
