Our view of the service

Our on-site assessment took place on 21 and 22 July 2026. Greenfields Care Home is a care home providing accommodation for people requiring nursing care, older adults and

people with a diagnosis of dementia.

The assessment took place due to the length of time since the previous assessment. We also reviewed information received about the service from partners and the public.

Audit and governance processes had not been sufficiently effective in identifying, monitoring, and driving improvement in relation to the consistency of person-centred interactions, the promotion of choice, implementation of specialist guidance, and aspects of record keeping. Whilst these issues were not reflective of the culture observed across the wider service, they highlighted the need for improvement.

People benefited from detailed, person-centred care planning, effective partnership working with healthcare professionals and a stable, knowledgeable staff team. Care records reflected people's preferences, life histories and individual needs, supporting staff to provide personalised care.

Relatives spoke positively about communication, involvement in care and the way staff responded to changing health needs. Staff were described as caring, approachable and skilled, particularly when supporting people living with dementia.

Management of the home had established a range of governance systems to monitor quality and safety, including regular audits, health monitoring systems, competency assessments and quality reviews.

There was evidence of learning, reflection and continuous improvement across the service, with management responsive to feedback and committed to developing the home further. Staff spoke positively about the leadership team and described an open culture where learning and development were encouraged.

Throughout the inspection, we observed many positive interactions between staff and people. Staff knew people well, communicated warmly and supported people with kindness and patience. Healthcare needs were monitored closely and people had access to a wide range of healthcare professionals, helping to support positive outcomes and reduce avoidable hospital admissions.

People's experience of this service

Feedback from people and relatives was found to be positive. However, some people felt staff were often very busy and did not always have sufficient time to spend chatting or to follow up non-urgent requests as quickly as they would have liked. One person told us, “The staff are too busy, they're run off their feet,” whilst also acknowledging the demands of the role. Another said, “They are doing a difficult job, and they are all underpaid and overworked.”

People described the home as a safe, caring and supportive environment where staff worked hard to meet individuals' needs. Relatives we spoke with consistently expressed confidence in the care provided, particularly for people living with dementia and complex health needs. Relatives praised staff for their kindness, patience and understanding. One relative told us, “[Person] is safe here, very much so,” while another told us, “I know they are safe and I know they look after [Person].”

Families told us staff knew people well, responded appropriately to concerns and kept them informed about changes in health and wellbeing.

People and relatives we spoke with told us staff supported people's dignity, choice and independence. Relatives described staff as respectful of individual routines and preferences, with one relative telling us, “[Person] gets to choose what time they go to bed, what time they get up and their meals.” People valued being encouraged to do as much as possible for themselves, with one person telling us, “They do let me do what I can do for myself. I like to be independent.”

Staff were praised for identifying and responding to healthcare needs, including changes in physical health and dietary requirements. One relative explained, “The staff picked up on the choking risk and dealt with it. [Person] has been having the diet they need to have.”

Relatives told us they felt communication was good and reported being contacted promptly when concerns became present. One relative told us, “If anything goes wrong, they are quick to contact us, doesn't matter what time of day,” and, “They are very helpful and tell me exactly what happened.”

Relatives spoke positively about opportunities to provide feedback and raise concerns, expressing confidence management and staff would listen and respond appropriately. One person told us, “We can very much make suggestions, and they will act on it.” Activities staff received praise for their efforts to engage people, with one relative describing them as “both brilliant.”

A relative told us, “I don't think there is anything they could do better. They do a marvellous job, they are lovely.” Another added, “The staff are lovely, can't think of anyone that isn't.” This identified a service where people feel safe, cared for and supported by a compassionate and dedicated staff team.
