Our view of the service

1-2-1 Live In care Ltd is a domiciliary care service that provides personal care and support to people living in their own homes. Not everyone who used the service received personal care. CQC only inspects where people receive personal care. This is help with tasks related to personal hygiene and eating. Where they do, we also consider any wider social care provided.

At the time of the assessment, the service supported 16 people who received personal care.

We carried out an assessment of the service which started on 23 May 2024 and was completed on 17 June 2024. This was the service’s first assessment since registration.

We reviewed information we had received about the service since its registration. We reviewed a range of records we requested from the provider. This included 5 people's care records and medicine records. We looked at 3 staff files in relation to recruitment and staff training, induction, and supervision. A variety of records relating to the management of the service, including policies and procedures were reviewed.

We spoke with 10 staff members, this included the registered manager, who is also the provider and the deputy manager who is also a business partner. We sought feedback from 3 people who used the service and 5 relatives. We also sought feedback from professionals working with the service.

The registered manager explained to us the extenuating circumstance which took place within the management team recently. We took this into account when considering what enforcement action was necessary and proportionate to keep people safe.

We have identified breaches of regulations in relation to safe care and treatment, safe recruitment, training and good governance.

We will request an action plan from the provider to understand what they will do to improve the standards of quality and safety. We will work alongside the provider and continue to monitor information we receive about the service, which will help inform when we next assess.

People's experience of this service

We found improvements were needed to ensure people's risks were assessed and mitigated in areas such as delegated nursing tasks, falls, skin pressure care and moving and handling tasks. Records relating to the assessment and management of people's risks required improvement to ensure staff had access to complete and up-to-date information to keep people healthy and safe.

Safe recruitment procedures were not always followed. Improvements were needed to ensure staff received training and appropriate assessments of their competence to fulfil their role and were supported through regular supervision and appraisal.

Improvements were also needed in relation to medicines management and quality monitoring systems.

We did not find that these shortfalls had impacted on people's care. The service took our assessment feedback on board and was starting to make the improvements we identified.

People were supported to have maximum choice and control of their lives, and staff supported them in the least restrictive way possible and in their best interests; the policies and systems in the service supported this practice.

Feedback we received from people and their relatives in relation to the service was positive.

Staff knew people well, were knowledgeable of people’s support requirement and needs and were passionate about delivering person centred care. They were also very positive about the support they received from the management team.

Infection prevention measures had been established and implemented. Staff had a good understanding of these procedures.

Comments from people and their relatives regarding the care and support received from the service included “They are a good firm and they know you and look after you. So of course—they are Wonderful!!” and “They are regular, consistent and reliable so without doubt I would recommend them, they enable mum to live her life as independent as possible. I would give them 10 out of 10 !!”
