Assessing needs

The provider did not consistently ensure people’s care and treatment were effective as care plans did not always contain sufficient information to guide staff consistently about how people needed to be cared for. For example, where people had specific health related conditions. The provider told us they would review these care plans immediately.

Delivering evidence-based care and treatment

Score: 3

The provider delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards. Staff supported people effectively, using various strategies, to make informed choices about what they wanted to eat and drink. People and relatives gave positive feedback about the quality and choice of the meals provided. A person said, “The food is excellent, there are plenty of choices.”

How staff, teams and services work together

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services. Staff described teamwork as positive and supportive.

Supporting people to live healthier lives

Score: 3

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support. People and relatives confirmed staff supported people to access healthcare services, when needed. A relative said, “They organise for the chiropodist and optician to come in.”

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves. People and relatives told us their experience of the care provided at Palmersdene had been positive. A person told us, “I couldn’t wish for a better place.”

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment. People confirmed they were able to make their own choices and decisions. Staff sought consent before providing any care. During lunchtime staff checked people wanted help first before providing assistance. A staff member said, “We always ask first before doing anything.”
