Staff and leaders actively listened to information about people who are most likely to experience inequality in experience or outcomes and tailored their care, support and treatment in response to this.

 

Staff demonstrated a strong understanding of the individuals they supported, and the assistance required to help people access services related to their care. There was a clear commitment to person-centred care, with staff tailoring their approach to meet each person’s needs and preferences.

Staff were supported through tailored training designed to help them recognise and respond to potential inequalities in access to care and treatment. This included training on equality, diversity, and human rights, equipping staff to identify when individuals might be at risk of disadvantage and to take appropriate, person-centred action.

To ensure continuity of care, the service arranged for health professionals, such as hearing specialists, opticians and chiropodists, to visit the home when individuals were unable to attend external appointments. This proactive approach helped prevent delays and ensured people received the care they needed in a timely manner.

Relatives spoke positively about the service’s responsiveness to changing needs. One relative shared how the service supported their family member in transitioning from a short-term respite placement to a permanent stay, They told us, “As soon as the 6-week respite care was coming to an end, Apple Trees worked with the local authority to get [Name] assessed for a permanent placement. It was all handled 100% safely, efficiently and in a caring manner.”
