Our view of the service

Date of Assessment: 20 March to 20 May 2025. The service is a residential care home providing support to older people, people living with mental health conditions, physical disabilities and dementia. We carried out this inspection to follow up on warning notices and in response to concerns we received over the safe handling of medicines from partner agencies. The provider did not have a good learning culture, the warning notices we issued in response to the last inspection, where there were breaches of regulation, had not been met. Incidents involving distressed behaviours and safeguarding concerns were not always investigated thoroughly. People were not protected from harm, nor were people safely monitored or supported. We raised 11 safeguarding concerns on behalf of people living in the home. People did not always receive their medicines safely and medicines were not always stored safely. Each person had their own care plan; however, care plans often contained conflicting information and were not kept updated with people’s changing needs and people’s care did not always reflect their needs, impacting upon their wellbeing. The provider did not always work effectively in partnership with other professionals or follow their recommendations. Staff told us they had not received regular supervision, although they felt supported by the [new] manager, and they told us the new manager was approachable. There were limited activities taking place and limited opportunities for people to engage in their interests or within the local community. People could not always live peacefully in their own home, some people targeted other people in the home without challenge from staff. Some audits were in place; however, these were not effective at identifying and acting on the concerns found during this inspection. Whilst the provider shared new systems and plans to improve the service, they had not acted promptly to concerns raised in the previous inspection and continued to place people at risk of harm.

We found continued breaches of regulation and a further breach of regulation at the service. We found 3 continued breaches of regulation, relating to safe care and treatment, person centred care and good governance. Due to similar concerns continuing around consent and new safeguarding concerns we found an additional breach relating to safeguarding people from abuse.

In instances where CQC have decided to take civil or criminal enforcement action against a provider, we will publish this information on our website after any representations and/ or appeals have been concluded.

This service is being placed in special measures. The purpose of special measures is to ensure that services providing inadequate care make significant improvements. Special measures provide a framework within which we use our enforcement powers in response to inadequate care and provide a timeframe within which providers must improve the quality of the care they provide.

People's experience of this service

Some people told us they felt safe living in their home. However, other people told us they did not feel safe living in their home. People told us there were enough staff on duty to support them as and when needed, they said they could raise concerns with staff. Relatives told us the provider shared information promptly when people’s health changed. Relatives told us they had observed health and safety checks being carried out and they said the home was regularly cleaned. People told us staff listened to them and treated them with respect. People said they received their medicines safely. People told us they could see a health professional such as a doctor when required. However, people told us they were often bored and there was limited opportunities for social engagement, 1 person told us they were lonely.

While the people we spoke to expressed that they were generally happy with their care, our assessment found care did not meet the expected standards.

We observed staff treating people kindly and warmly. We observed staff knocking on people’s bedroom doors and asking permission before entering. However, we also observed people shouting and threatening other people in the home. There were occasions where people were not always supported safely and 1 occasion where staff did not respond kindly to a person in distress. We shared our observations with the provider for investigation and the local authority safeguarding team.
