Safe
Safe – this means we looked for evidence that people were protected from abuse and avoidable harm. At our last assessment we rated this key question as good. At this assessment the rating has remained as good. This meant people were safe and protected from avoidable harm.

Learning culture
The provider had a proactive and positive culture of safety, based on openness and honesty. Staff listened to concerns about safety and investigated and reported safety events. Lessons were learnt to continually identify and embed good practice.

We did not look at Safe systems, pathways and transitions during this assessment. The score for this quality statement is based on the previous rating for Safe.
Safeguarding
The provider worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. Staff concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect. The provider shared concerns quickly and appropriately.
Involving people to manage risks
The provider worked with people to understand and manage risks by thinking holistically. Staff provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them.
Safe environments
The provider did not always ensure environmental risks were formally assessed, documented and managed, to provide assurance that people were protected from avoidable harm and that the environment supported the delivery of safe care.
We observed areas of the garden environment that presented potential trip and fall hazards, including uneven paving slabs, raised manhole covers, a steep incline and stairs leading to a fire escape. There was no documented risk assessments to demonstrate how these risks were being managed. We raised this with the registered manager who took prompt action to address the concerns identified.
The service had previously received a fire safety visit from the local fire authority, which identified actions for the provider to complete. We found the provider had made progress and commissioned external contractors to undertake the remaining remedial works. However, one action relating to the condition of fire doors remained outstanding. Although interim control measures were in place, there was no documented risk assessment or action plan to demonstrate how associated risks were being managed pending completion of the works. Following the inspection, the provider completed a risk assessment and action plan to support the safe management of risks during the remedial works.
We identified an environmental concern that did not align with current Health and Safety Executive (HSE) guidance. This was discussed with the provider and registered manager, who acted immediately to address the issue and reduce potential risks to people using the service.
Safe and effective staffing
The provider made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care that met people’s individual needs.
Infection prevention and control
We did not look at Infection prevention and control during this assessment. The score for this quality statement is based on the previous rating for Safe.
Medicines optimisation
The provider made sure that medicines and treatments were safe and met people’s needs, capacities and preferences. Staff involved people in planning, including when changes happened.
