Responsive
People were involved in decisions about their care. The service provided information people could understand. People knew how to give feedback and were confident the service took it seriously and acted on it. The service was easy to access, and people received fair and equal care and treatment.

Person-centred Care
The service made sure people were at the centre of their care and treatment choices and they decided, in partnership with people, how to respond to any relevant changes in people’s needs.
Care provision, Integration and continuity
The service understood the diverse health and care needs of people and their local communities.
Providing Information
The service supplied appropriate, accurate and up-to-date information in formats that were tailored to individual needs.
Listening to and involving people
The service made it easy for people to share feedback and ideas, or raise complaints about their care, treatment and support.
We saw a complaints process was in place. There was one complex complaint in the last 18 months that had been investigated and managed appropriately.
Equity in access
The service made sure that people could access the care, support and treatment they needed when they needed it.
Equity in experiences and outcomes
Staff and leaders actively listened to information about people who were most likely to experience inequality in experience or outcomes and tailored their care, support and treatment in response to this.
The practice had lower cervical screening and child immunisation rates compared to national averages. The practice leaders provided an explanation that due to the diverse ethnicity of patients, there were lower uptake rates of screening and immunisation. They had had a plan to address this such as using various communication methods with patients in many different languages. The plan also included lead roles to ensure patients were followed up with a view to increasing uptake rates.

People were supported to choose appropriate treatments.
