Our view of the service

Gracefield Health Care Limited (GHC) - 31 St Domingo Grove is a residential care home providing accommodation and personal care for up to six people. We assessed the service to follow up on enforcement that was in place.

The service is located in a residential area of Anfield, Liverpool, with accommodation arranged across four floors. The service supports people with a learning disability, autistic people, and individuals with mental health support needs. At the time of the assessment, six people were living at the service.

We assessed the service against ‘Right support, right care, right culture’ guidance to make judgements about whether the provider guaranteed autistic people and people with a learning disability respect, equality, dignity, choices, independence and good access to local communities that most people take for granted.

Care plans and risk assessments were in place and provided staff with clear guidance on how to support people safely. Although some care plans did not consistently evidence people’s involvement, staff and relatives told us that people were actively engaged in planning their care. The service had systems in place to monitor accidents and incidents, identify learning, and share information appropriately.

The service worked with a multidisciplinary team to ensure people’s needs were met. People had meaningful and personalised goals, which were reviewed regularly. Mental capacity assessments and best interest decisions were completed in line with the Mental Capacity Act. Any restrictions in place were done lawfully and reviewed. People were encouraged to maintain their independence and had individual timetables, enabling regular access to the community.

People were supported by staff who knew them well, and we observed positive and respectful interactions between staff and those living in the service. Meetings with people, relatives and staff were used to share information and drive improvements.

Quality assurance systems were in place and provided detailed oversight of the service. These systems supported effective monitoring, continuous improvement and ensured the service operated safely.

People's experience of this service

We gained peoples feedback through discussions with peopleandrelatives and from observations.

We saw lots of positive and friendly interactions between people and staff during our visit. Staff spoke with people in a warm and relaxed way, and people seemed comfortable around them. It was clear that staff knew people well, including what they liked, their routines, and the best way to support them.

Overall, people told us they were happy living at the service and felt well supported by staff. They spoke positively about their daily routines and the activities they enjoyed, such as playing pool, going to football matches ,knitting and baking. People were encouraged to take part in things that mattered to them and to continue their own hobbies and interests.

Relatives also gave positive feedback about the service. They described staff as kind and caring, saying things like “They are angels the way they look after [loved one]”and, “Staff are amazing, they are so good. ”Relatives told us communication was generally good, and that staff contacted them if anything changed. One relative said, “They communicate well. If anything is wrong with [loved one], they would ring us.” However, one relative expressed concern about the level of communication when the registered manager was not present.

Several relatives said they had noticed improvements in the service since the last inspection, with one commenting, “There has been lots of improvement.” Overall, people and their relatives felt confident in the care and support provided.
