Staff told us that care plans were devised and reviewed by the multidisciplinary team, and that patients received personalised care and treatment. However, not all patients who were able to, had been involved or seen their plan of care. The provider had a designated physical health team in place, who contributed to regular monitoring for certain physical conditions, and staff implemented relevant care plans around individual physical healthcare needs. Staff told us that the loss of ward based social workers had impacted on patients, including the facilitation of discharges, liaison with external safeguarding teams and liaison with the Ministry of Justice.
