Our view of the service

Date of assessment: 9 to 13 June 2025.

Milverton Nursing Home is a care home providing nursing and personal care to older adults, many of whom are living with dementia. At the time of our assessment 28 people were using the service.

We undertook this assessment to follow up on a previous rating of requires improvement for the key questions safe and well-led. At this assessment we found the rating remained requires improvement. The provider was in breach of the legal regulations relating to safe care and treatment, and good governance.

Safe medicines management was not in place. Excess stocks of medicines were held at the service and accurate stock counts were not maintained. Protocols for ‘when required’ medicines were not always in place. Care records did not contain complete and accurate risk assessments and risk management plans. Environmental risks, including the risk of falling from height, were not adequately mitigated.

Governance systems were not always effective to identify and timely action areas requiring improvement. Some systems had not been adequately maintained and kept up to date. Action was taken after our site visit to respond to the feedback provided and start to address the concerns raised.

Action had been taken following our last assessment to address concerns around recruitment practices, handling of incidents and accidents, the duty of candour and ensuring a registered manager was in post.

Safe staffing levels were maintained and staff had access to regular training opportunities. Staff felt well supported in their role and able to speak openly to their management team.

We have asked the provider for an action plan in response to the concerns found at this assessment.

People's experience of this service

We received mixed feedback from people and their relatives. The majority were happy living at Milverton Nursing Home. Comments included,“It’s a really good place - very caring and professional,” and “My relative has lived here for 15 months. I wouldn’t want them to live anywhere else now. We are happy with things.” However, some people told us there had been some staff turnover in the year preceding our assessment and they felt this had impacted their ability to speak openly and honestly with staff. They felt the management team were not always accessible and approachable.

Some people and relatives felt the home was often short of staff on a weekend, however, the rotas we viewed showed the home was staffed appropriately. We observed people being supported with moving and handling in a careful and considerate manner. Staff spoke to people with kindness. People and their relatives felt safe at the service and were happy with the support they received to minimise any risks to their health.
