Assessing needs
The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them. Monitoring and improving outcomes
The provider monitored all people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they fully met both clinical expectations and the expectations of people themselves. Observations confirmed staff were consistently talking about people’s health needs and sourcing appropriate health care as needed and where further investigations were required this was arranged. Records evidenced a proactive approach to healthcare, with prompt referrals and appointments arranged to ensure people received timely support and achieved the best possible health outcomes. We observed a thorough handover where the information provided was comprehensive and person-centred. Staff shared detailed information about people’s wellbeing, identified risks, and support needs, enabling continuity of care and ensuring staff were equipped to respond appropriately throughout their shift.
Consent to care and treatment
The provider always carefully explained to people what their rights around consent were, made sure they fully understood them and always fully respected these when delivering person-centred care and treatment.
