During an inspection looking at part of the service

During our previous visit to the home in November 2012 we identified concerns about the systems in place to manage the control and spread of infection. We also identified concerns regarding the environment. We found that the provider had failed to carry out actions required by other regulators who had visited. This was a follow up visit to check that the provider had carried out the improvements recorded in their action plan.

We found that the home had implemented systems to monitor infection and they had carried out a number of audits to ensure that good standards of infection control were in place. These systems were in the early stages of development but the manager had clear plans in place to complete this work and to ensure that regular auditing continued.

We found that improvements had been made to the environment which included carrying out recommendations from both the fire officer's visit and the environmental health agency. We also found that electrical wiring work had been completed. We did note that some improvements to the environment were still required and agreed with the provider that they would send us an annual plan to demonstrate how and when this work would be completed.

During a routine inspection

People were supported to make decisions and choices regarding their care and treatment. Where people were unable to consent the provider acted in accordance with legal requirements. We observed staff supporting people to make decisions.

People said they were well cared for and liked living at the home. One person said "It's a nice place to live." We carried out some observations and found that people experienced positive interactions from staff working at the home. However we also observed people being left unsupervised which meant that they may not receive appropriate support when needed.

The premises required attention as reports written from other professionals had not been actioned which meant that people could be at risk. This included reports from the fire officer, environmental health officer and the electrical engineer. All raised issues that related to the environment. We also found that there were some unpleasant odours present in some areas of the home which need to be addressed so that it is a pleasant place for people to live and work.

The home had suitable equipment available to support people living at the home and this equipment was well maintained to ensure that it was safe to use.

Staff were appropriately recruited with relevant checks being completed. Staff told us that they enjoyed working at the home.

There were systems to support people who had concerns or complaints and we could see that complaints were dealt with appropriately.

During a routine inspection

We carried out this review as part of our normal inspection programme, however due to a concern which was made to us about people not being provided with a choice about when they rose from bed, we brought forward the inspection.

We were unable to talk with some of the people living at Abbey Lodge due to their illness. So we observed how they interacted with the staff to see what choices the staff enabled them to make and we talked to the relatives and friends who visited them.

We talked to four relatives who told us they had been fully informed and were involved in the planning of people's care. They explained the staff encouraged and enabled people to make day to day decisions about their daily lives, such as choice of menus or when to rise from or retire to bed. They told also us people were treated with dignity and respect by staff.

We saw when people interacted with staff they smiled and appeared comfortable. Staff were observed approaching people in a friendly manner and talked with people in a way which showed they understood their care needs. Relatives told us the staff were 'patient' and 'brilliant'.

One relative told us the home had improved 'dramatically' and the home was 'lovely'. They explained up until recently there had been many activities taking place in the home, such as snakes and ladders, connect four. They said 'There was always something going on.'

Relatives said staff were approachable and confident any concerns they raised would be responded to.
