Well-led
Well-led – this means we looked for evidence that service leadership, management and governance assured high-quality, person-centred care; supported learning and innovation; and promoted an open, fair culture. At our last assessment we rated this key question good. At this assessment the rating has remained good. This meant the service was consistently managed and well-led. Leaders and the culture they created promoted high-quality, person-centred care.

Staff demonstrated an understanding of the service’s vision and values and were able to describe these in practice. However, these were not consistently embedded within the staff culture or experience. Feedback from staff was mixed, with some reporting they felt well supported and positive about their role, while others did not feel consistently valued or listened to. These concerns were shared with the registered manager at the end of the assessment to support improvement.

Governance, management and sustainability
The service had governance systems and a management structure in place to support oversight and delivery of care. However, these were not consistently effective in identifying and addressing areas for improvement. For example, current auditing processes did not identify the required improvements in relation to risk assessments and the recording of the application of prescribed medication patches. Although immediate action was taken to address this during the inspection, systems will need to be strengthened to ensure audits reliably identify and drive improvement.

Learning, improvement and innovation
The provider focused on continuous learning, innovation and improvement across the organisation and local system. They encouraged creative ways of delivering equality of experience, outcome and quality of life for people. They actively contributed to safe, effective practice and research.
