Our view of the service

Date of inspection: 9 and 10 June 2025. The inspection was carried out to check if improvements had been made following our last inspection.

Spencefield Grange is a residential service providing support to older people living with dementia, mental health conditions, and physical disabilities. The provider was previously in breach of the legal regulations in relation to safeguarding, safe care and treatment, staffing, safe management of medicines and governance. Improvements were found at this inspection and the provider was no longer in breach of these regulations.


At the time of our inspection there was not a registered manager in post. A new manager had been in post since November 2024 and had submitted an application to register. We are currently assessing this application.

The provider had a good learning culture and people could raise concerns. The manager investigated incidents thoroughly. People were protected and kept safe. Staff understood and managed risks. The facilities and equipment met people’s needs, were clean and well-maintained and any risks identified were mitigated.

There were enough staff with the right skills, qualifications and experience to care for people. Managers made sure staff received training and regular supervisions to maintain high-quality care. Staff managed medicines well. Staff made sure people understood their care and treatment to enable them to give informed consent. Staff involved relevant people and made decisions in people’s best interests where they did not have capacity. Staff responded to people in a timely way.

The provider supported staff well-being. Staff provided information people could understand. People and relatives knew how to give feedback and were confident the provider took it seriously and acted on it. Leaders were visible, knowledgeable and supportive. Managers were receptive to new ideas. There was a culture of continuous improvement.

People's experience of this service

Most people were positive about the quality of their care. They felt safe and were involved in planning their care where appropriate and understood their rights. People said they received care from knowledgeable staff who treated them as individuals. One person said, “Staff treat me well.” People and relatives felt able to complain and were confident action would be taken, and improvements would be made. People were involved in decisions about their care. People had choice in their care and were encouraged to maintain relationships with family and friends. People were treated with kindness and compassion. People had enough to eat and drink to stay healthy. Relatives spoke highly of the improvements made at the service since the last inspection. Comments included, “The care owner has improved the home massively over the last year with new management, it is brilliant now.” Another relative commented, “There has been a change in the management structure and the staff are now more aware of their responsibilities towards the residents therefore I do feel that my [relative] is kept safe. I feel much happier these days.”
