Well-led – this means we looked for evidence that service leadership, management and governance assured high-quality, person-centred care; supported learning and innovation; and promoted an open, fair culture. At our last assessment we rated this key question as good. At this assessment the rating has remained as good. This meant the service was consistently managed and well-led. Leaders and the culture they created promoted high-quality, person-centred care.

Shared direction and culture

The provider had a shared vision, strategy and culture. This was based on transparency, equity, equality and human rights, diversity and inclusion, engagement, and understanding challenges and the needs of people and their communities.

Governance, management and sustainability

Score: 2

The provider did not always have clear responsibilities, roles, systems of accountability or good governance. They did not always act on the best information about risk, performance and outcomes, or share this securely with others when appropriate.

The provider had systems and processes to review and monitor quality in the service; however, the issues identified throughout the inspection had not been identified by the registered manager or the quality assurance processes in the care home.

Areas we identified needed improvement were around effective deployment of staff, ensuring all key areas of training were in place, and ensuring the environment was safely managed. Although the provider has taken action to address these improvements, we have not been able to assess the impact or sustainability of those improvements.

The provider carried out regular quality checks of the service and developed a tracker to monitor improvements needed. The registered manager had completed their own service improvement plan, and clinical actions were noted in the clinical review meetings. We discussed with the management team our concerns relating to actions recorded in different improvement plans and trackers and how there is a risk of oversight being lost in some areas and improvements not being made. The provider acknowledged this and committed to develop one oversight improvement plan to streamline and better monitor improvements in the future. They also acknowledged the need to share their improvement plan with staff and other relevant stakeholders, including people and relatives.

Additionally, although the clinical team carried out analysis of incidents, wounds and looked for trends, additional detail was needed to ensure these robustly reviewed those areas. The clinical lead told us they would look for patterns in trends and recurring themes to proactively address emerging risks to people. They developed and implemented an enhanced audit tool during this inspection.

People, staff and relatives were very positive about the management team and the relatively new registered manager. They told us about changes that had been put in place since their appointment, such as increasing staffing, developing training and improving the culture of the home. One staff member said, “[Registered manager] is lovely, they listen, if I have any problem I can go to them and they will help. Every morning they go to each client’s room to greet them, I think that’s nice. [Clinical lead] is lovely, just so clever, any problems they will try to help as well.” One person said, “The manager, I think, leads by their own example. They are always on the floor, watching, checking. I think [Registered Manager] is a good manager and a breath of fresh air.”
