Safe – this means we looked for evidence that people were protected from abuse and avoidable harm. At our last assessment we rated this key question as good. At this assessment the rating has remained as good. This meant people were safe and protected from avoidable harm.

Learning culture
The provider had a proactive and positive culture of safety, based on openness and honesty. Staff listened to concerns about safety and investigated and reported safety events. Lessons were learnt to continually identify and embed good practice.

We did not look at Safe systems, pathways and transitions during this assessment. The score for this quality statement is based on the previous rating for Safe.
Safeguarding
The provider worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. Staff concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect. The provider shared concerns quickly and appropriately.
Involving people to manage risks
The provider worked with people to understand and manage risks by thinking holistically. Staff provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them.
Safe environments
The provider detected and controlled key environmental risks, including fire safety and hazardous substances in the care environment and made sure equipment, and technology supported the delivery of safe care. Although individual environmental risk assessments were not in place, people were supported to raise concerns about their living environment and escalate these where necessary to help maintain their safety and wellbeing.
Safe and effective staffing
The provider ensured there were enough suitably recruited staff with the skills and knowledge required to meet people's needs. Recruitment processes were robust, and staff received training, supervision and support to carry out their roles effectively. Staff told us they were encouraged to keep their learning up to date and worked well together to provide safe care. While not all staff had completed the mandatory training required to support autistic people and people with a learning disability, the provider acknowledged our feedback and had begun to address this by sourcing training opportunities for staff to complete.
Infection prevention and control
We did not look at Infection prevention and control during this assessment. The score for this quality statement is based on the previous rating for Safe.
Medicines optimisation
The provider made sure that medicines and treatments were safe and met people’s needs, capacities and preferences. Staff involved people in planning, including when changes happened. Medicines were generally managed safely, and people received their medicines as prescribed. Although we identified some recording errors on a Medication Administration Record (MAR), there was no evidence of harm or adverse impact to people as a result. The service had already identified these issues and was able to demonstrate that appropriate action had been taken to support staff learning and reduce the risk of recurrence. Subsequent MAR records reviewed showed that the errors had been addressed and recording had improved.
