Our view of the service

Date of Inspection: 20 and 22 January 2025. The service provides support to adults of all ages, some living with dementia. We found 5 breaches of the legal regulations in relation to safe care and treatment, staffing, safeguarding people, person centred care and good governance. At our last inspection we found significant concerns around risk management and the provider was issued warning notices for regulations 12, 17 and 18. We found continued breaches of regulation during this assessment. People had not always been protected from the risk of harm. Staff lacked the knowledge and competency to support people. People at risk of dehydration, constipation, falls, skin breakdown, distressed behaviours, and choking were not well supported. Medicines were not managed safely. Guidance and documentation was poor. There was a poor culture at the service which the new manager was trying to improve. Governance and oversight was ineffective. An inspection has been undertaken of a specialist service that is registered for use by autistic people or people with a learning disability. At the time of the assessment, the service was not used by anyone with a learning disability or an autistic person. However, we assessed the care provision under Right Support, Right Care, Right Culture, as it is registered as a specialist service for this population group.

This service is being placed in special measures. The purpose of special measures is to ensure that services providing inadequate care make significant improvements. Special measures provide a framework within which we use our enforcement powers in response to inadequate care and provide a timeframe within which providers must improve the quality of the care they provide.

People's experience of this service

People experienced poor care and outcomes and had been exposed to harm. Some people had communication barriers which were not well supported which caused further distress to the person. People did not benefit from a service which had a well embedded positive culture. People’s care was not delivered in a holistic or person-centred way. Staff had no information about people’s life histories, particular likes or interests, the manager identified this was an area that required further work and consideration. Governance systems were not effective, and people were at risk of inappropriate care and support. We received some positive feedback from relatives and people about the service. However, professional partners shared concerns with us about the service and the effectiveness of its leadership in sustaining or embedding good practice or improvement so people’s outcomes improved.
