Systems and processes were embedded, however some improvements were required.

There were effective processes for identifying and managing risks, investigating incidents and accidents, and for receiving and acting on safety alerts.

The practice had systems and processes for learning, quality assurance and continuous improvement. This included undertaking audits according to recognised guidance.

The practice told us that staff meetings were not currently being held; these were due to be reinstated in February 2026 which would help with improved communication between the practice and the group.

Concerns and complaints were responded to appropriately, and outcomes were discussed to share learning and for improvement. Improvements should be made to the monitoring of complaints. The practice told us that a new system was being implemented to facilitate better recording of complaints.

Staff had clear responsibilities, and systems of accountability to support good governance.
