Change your registered provider address or location address, for a domiciliary care service only.

Check if you should use this form

You must not use this form if any of these apply:

  • you need to change service user bands or service types
  • managers need to change any of their details
  • you have submitted other applications to make changes to your registration and these have yet to be decided
  • we have started enforcement action against you
  • you are registered for both domiciliary care service (DCC) and supported living services (SLS)

Registered managers

Your registered manager does not need to apply separately. If we change your domiciliary care service address we will update your managers registration automatically.

Update your statement of purpose

You must send an updated statement of purpose with your applications. This needs to reflect the new business and location address.

Get it right first time

Help us to deal with your application more quickly:

  • refer to your Certificate of Registration when completing your forms. Most applications are rejected because the provider or location details are incorrect
  • submit all of the supporting applications at the same time
  • make sure you send us an updated statement of purpose
