Serious injury to a person using the service – notification form
Submitting notifications during the coronavirus (COVID-19) outbreak
You must send your notification by email or submit it online. Do not send it by post. This is because our offices are closed and our staff are working from home.
You must tell us about a serious injury to a person using your service if either of the following has happened:
- the person was seriously injured while a regulated activity was being provided
- their injury may have been a result of the regulated activity or how it was provided
If the serious injury is the result of an assault, you should use our allegation of abuse notification form instead.
If you have an account with the CQC Provider Portal, you can send us your notification online.
Notify us by email
If you do not have access to the Provider Portal, you can use this form to tell us about a serious injury to a person using your service.
Send us the form
email the completed form to HSCA_notifications@cqc.org.uk.
