Use this form to cancel your manager registration for all the regulated activities you manage.

Who can use this service

You can only apply to cancel your manager registration using this form if you:

  • are a registered manager
  • manage regulated activities for a provider with more than one location.

Who cannot use this service

There are different services if you want to:

You cannot cancel your registration if we have issued a notice to you.

What happens next

Once you submit your application, we will review it and confirm our decision.

If we reject or refuse your application, we will let you know why we have come to this decision.

You could be fined up to £2,500 if you give false or misleading information

If you submit your application too far in advance of the effective date we may return it. You will need to submit it closer to the date of change.
