How to apply

If you have an account with the CQC Provider Portal, you can send us your application online.

If you don't have access to the Provider Portal, follow this process:

Tell us about the location you want to add

Use this form to add the first or only location.

Add a location: Provider application

If you're applying to provide a residential service for people with a learning disability or autism at this location check you meet the requirements: Right support, right care, right culture.

You will then need to complete an additional form with your application. This will show how you will meet the requirements for these services.

From 11 November 2021, providers and registered managers of care homes of ‘accommodation for persons who require nursing or personal care’ must comply with vaccination as a condition of deployment (VCOD) requirements when you vary your registration.
