If you have an account with the CQC Provider Portal, you can send your application online.

Who should use this form
Use this form if you're a registered manager who has registered with CQC and you want to make changes to conditions placed on your personal registration.
For example, you might be a registered manager responsible for three regulated activities at a GP practice. When you registered with CQC, we may have placed a compliance condition on your registration. This condition may require you to complete training in the safe handling of medicines by the end of a three-month period. When you’ve completed your training, you will need to apply for us to lift the condition.
You may need to submit supporting evidence with your application. For example, proof you have obtained a required qualification.
